Person-Centered Care: Pelajaran Tentang Manusia dari Seorang Ibu dan Dua Sisir Pisang

28 Juli 2018 adalah bulan ke-6 saya bertugas sebagai dokter gigi Pegawai Tidak Tetap (PTT) di sebuah Puskesmas Pedesaan di Kabupaten Lombok Utara, Nusa Tenggara Barat. Menjelang kelulusan sebagai seorang dokter gigi, berbagai gambaran ideal mengenai kemana langkah ini akan dibawa selalu memenuhi kepala hingga terasa sesak. Mimpi untuk dapat melanjutkan sekolah ke luar negeri serta menempuh jurusan manajemen kesehatan atau kesehatan masyarakat agar dapat memberikan impact yang luas bagi negara tercinta, pernah terlintas di pikiran. Namun, tanpa disangka, datanglah kesempatan untuk mengabdikan diri di daerah, yang dapat dikatakan pelosok, jauh dari ambisi dan gemerlap kota metropolitan. Segala keunggulan pendidikan di Universitas terkemuka di negara maju pun jauh dari bayangan. Namun, justru momen inilah yang membuka mata saya tentang makna menjadi manusia yang berguna bagi manusia lain dimulai dari lingkungan terdekat, dimulai dari saat ini, dimulai dari tanah air tercinta. 


Juni 2018, kali pertama Ibu Baiq datang ke puskesmas tempat saya bertugas dengan keluhan yang memerlukan perawatan yang terbilang cukup rumit sehingga tidak bisa saya lakukan di Puskesmas. Merujuk Ibu Baiq untuk mendapatkan perawatan di fasilitas kesehatan dengan sarana yang lebih lengkap dan dokter gigi yang lebih berkompeten kemudian menjadi satu-satunya pilihan. Tak sebentar waktu yang saya perlukan untuk dapat  memberikan penjelasan kepada Ibu Baiq mengenai kondisi penyakitnya, pilihan perawatannya, langkah-langkah perawatan, hingga komplikasi pasca perawatan yang dapat terjadi. Ibu Baiq akhirnya setuju untuk dirujuk ke fasilitas kesehatan di Kota Mataram.

Sekitar 10 hari setelah saya bertemu Ibu Baiq untuk kali pertama, beliau kembali menyambangi  Poli Gigi di puskesmas tempat saya bertugas. Di hari tersebut, beliau mengeluhkan beberapa rasa tidak nyaman yang dirasakan selama masa penyembuhan pasca perawatan. Rasa tidak nyaman ini wajar dialami oleh pasien selama masa penyembuhan karena masih terjadi proses peradangan yang harusnya akan segera membaik dalam beberapa hari kemudian. Saya tenangkan Ibu Baiq dan perlahan saya coba jelaskan mengenai proses penyembuhan yang sedang dialami oleh tubuhnya, “Tenang saja, Bu. Bekas lukanya Alhamdulillah kondisinya saat ini baik. Wajar jika masih merasa sakit dan sulit membuka mulut, karena masih dalam proses penyembuhan. Yang penting Ibu tetap melanjutkan minum obat yang diresepkan dan latihan membuka mulut, Insya Allah dalam tiga sampai tujuh hari lagi akan segera membaik.” Beliau kemudian kembali merasa tenang dan pulang ke rumah dengan raut wajah yang lega.

 

Beberapa hari kemudian, saya terkejut saat melihat daftar pasien yang mendaftar ke Poli Gigi karena lagi-lagi ada nama Ibu Baiq. Satu yang saya takutkan, bahwa penyembuhannya tidak terjadi dengan baik. Namun, ternyata beliau nampak segar dan baik-baik saja. Kemudian saya persilakan beliau untuk duduk di kursi periksa, saya bertanya “Ada keluhan apa, Bu? Bagaimana sekarang kondisinya?” yang kemudian beliau jawab “Alhamdulillah sekarang sudah tidak terasa sakit sama sekali, Dok, saya bisa membuka mulut normal seperti biasa.” 

 

Kemudian refleks saya menjawab,“Wah saya kaget lho, Bu, tadi melihat nama Ibu di daftar pasien. Saya kira ada keluhan lagi,” Beliau menjawab dengan tawa sambil malu-malu berkata, “Hehehe, cuma pengen kontrol aja, Dok.”

 

Ibu Baiq pun kemudian izin keluar sebentar dari ruang periksa dan kembali dengan membawa dua sisir pisang. “Ini lho, Dok, cuma mau kasih ini, saya ambil dari kebun rumah saya. Terima kasih ya, Dok, sudah membantu merawat saya.” Betapa kagetnya saya, ternyata Ibu Baiq sengaja datang kontrol kembali untuk memberi saya pisang yang beliau ambil dari kebun rumahnya.



Satu Momen di Puskesmas 

 

Tidak menyangka bahwa momen sederhana itu kemudian menjadi momen yang menyadarkan saya betapa pentingnya memperlakukan pasien sebagai manusia, yang tidak seharusnya hanya dinilai berdasarkan penyakitnya, namun sebagai manusia yang juga memiliki cita-cita, pilihan, kebutuhan, serta kemampuan yang melekat dalam dirinya.


Pertemuan itu kemudian menjadi awal dari lahirnya mimpi dalam diri saya untuk bersama-sama merangkul, mendorong, dan memberi energi bagi seluruh penyedia layanan kesehatan agar bersama-sama dapat memberikan layanan kesehatan yang selalu berpegang pada prinsip bahwa pasien adalah manusia yang lebih dari penyakitnya. Person-centered care kemudian menjadi fondasi dari mimpi ini. Sebuah prinsip pelayanan kesehatan di mana pasien bersama dengan  penyedia layanan dan profesional kesehatan bersama-sama menjadi partner dalam rangkaian perawatan untuk mewujudkan kualitas hidup yang lebih baik. Sebuah prinsip yang akhirnya menjadi benih dari lahirnya TumbuhSehat.

A Moment to Reflect

It's been exactly a year since the last time I posted my last writing. I still write, even if it's only in the form of Instagram's caption... I wrote my last writing on Ramadhan. And now, one year later, it's almost the end of Ramadhan again. This year is different though, and peculiarly better, because I finally can spend more time at home with my... um... spouse, getting closer to God, sadly due to COVID-19. 

Turns out that the last year I still can't get over my problems. I still drowned in it and still can hardly breathe. But, despite all the problems which makes me so demotivated at times, I am feeling blessed  to finally be able to be closer, literally and metaphorically, with my spouse. Eventhough our journey might not be easy, well I guess we will always find our way back to each other. Well "Time can't erase a feeling this strong ♪"





Fortunately due to COVID-19 I got more time to think, to reflect and to balance my life. To be closer to God. To have deep conversation with my spouse. And to try better to heal from all the wound that I get. Time to move on now, I guess. Get up and live my life a little bit better and happier in each and every day. 

I used to say writing is my sanctuary. But how could I forget my sanctuary these past year? Starting from this post, I really wish (and will do any effort needed) to write regularly, again. 

Oh and one more good news, I started my residency at Department of Conservative Dentistry, University of Indonesia since last August 2019. Hope all goes well and may the best of todays be the worst of tomorrows. Let us heal together, dear Universe.

Tat Tvam Asi

I really can not believe that I haven't written anything since January. Well, there are some drafts I left undone and which I haven't touched again since the last time I wrote them. What gets in the way? Life does. Got caught up taking care of documents before I ended my job at underdeveloped area in North Lombok, West Nusa Tenggara. Got to move back to my hometown for a while before moving again to Indonesia's dazzling capital city, Jakarta. And not to mention, as always, a little twist here in there. I got so caught up I forget to reflect. 

Writing has always been a medium of reflection for me. Writing my thoughts magically can make my scattered thoughts lining in a nice order thus I can consider matters in a deeper way. Today, for the very first time after what felt like forever, I feel inspired again. Done with cooking my meal for sahoor, done replying chats I should have replied weeks ago, done tidying my room, done learning things I'm supposed to learn, and about to meet one I love this end of day, I feel calm yet alive. And I feel like I want to write.

Last February was my last month working as a dentist in a primary health care in North Lombok, West Nusa Tenggara in an underdeveloped and post earthquake area. I chose to serve in an underdeveloped area because I want to know what it's like for the rest of society live outside of the sparkling metropolitan cities, I want to know what they need and their health condition and I want to serve them [will write about it in a separate post about PTT (health workers work in underdeveloped area)], but primarily it is because I got sick of life in the city full of materialism. 

I too, used to dream of continuing my education abroad on the top 10 world universities under the reason of learning what is best for my country. While actually there is a part of me that only want to study abroad for the sake of enjoying a cup of hot coffee in one corner of public park in the developed country. There, then, I realised how can I contribute to my country if I didn't even know how the majority live in our country, struggling even to get proper electricity? 

So, to North Lombok off I go. Better than my expectation, there, I did not only realise how beautiful Indonesia is, given the heavenly landscapes of hills and beaches of Lombok, but I also get to experience humanity in a closer and deeper way. Before the 7 SR earthquake in North Lombok, even if the infrastructure of the area is already developed (paved road, proper irrigation, easy access to clean water), its human resources' quality is far behind what we have in Java, Indonesia's most populated island. 

To be able to speak with elementary students during my dental health education programs, asked them about their dream, and share with them some inspiring stories, to be able to serve society from the middle low class in the primary health care, I feel like this is how we actually should have lived, by giving and treat others in a best way possible, for what we give is what we get. I feel like the more we share and give back to society, the more happiness we will get.

Also in Lombok, I met my new families; Ibu Nengah's family who own the place I lived for my 13 months stay and my fellow doctors from different cities who work there. From Ibu I also learn a lot, about kindness, families, love, and humanity.

So when I have to move back to the city, at first I feel like I get uninspired. In Lombok I am able to enjoy the slow living, a balance work/life all because of the easy access to fresh air, open spaces, and beautiful hills and beaches unlike here in Jakarta, I worked in underdeveloped area and is easy to share my knowledge and using my expertise to help others. Tat tvam asi; I am you and you are me. In Lombok, I live so close to this belief, far from materialism. And it makes me happy.

With some twist also get in the way, I forget to reflect and to be grateful. Thanks god, this month is Ramadan, a month full of reflection and duties to move us closer to God. Asking the staff of my rented room about whether or not she has food for iftar, giving food for them, although not much, actually is also a simple act of applying Tat tvam asi in each and everyday. Isn't happiness is within ourselves? No matter how hard things get and no matter where we at, when we always create positive mindfullness and giving our best attention and treat others the way we want to be treated, wouldn't life will treat us the same way too?
